Fall 2017 Registration Forms
For Fall 2017, please use online registration form. There will be one in-person registration day on September 2nd, from 9:30am -11:30 am, in the Azusa Pacific University music building (aka Warren Music Center on the corner of Citrus and E. Alosta). Room number will be posted on signs.
Scholarship applications for Fall 2017 are due by September 2nd.
All forms can be printed and filled out, then returned via regular mail to the address listed here, sent via email to email@example.com or delivered to Dr. Koops in the APU School of Music. If Dr. Koops is not on campus, please leave them with the receptionist in the School of Music Office, located just inside the front doors of the building.
